Apparently, the producers felt the show needed a cast shake up and, to be fair, the character House noted in Alone that it was strange for him to have had exactly the same staff for three straight years. In the Season 1 episode DNR, Foreman admits that he is on a three year contract, which would have ended at about the end of Season 3. When Dr. Cate Milton (Mira Sorvino), a psychiatrist trapped in the South Pole and the research station's only doctor, becomes ill in the middle of her assignment, she and House are thrust into a long-distance relationship of sorts.
